Celvol Sample Request Form (except Celvol E and Celvol SP)
Complete the form below to request up to 3 different samples of Celvol Polyvinyl Alcohol. The standard sample size is 1 lb (pound) for solid products and 1 qt (quart) for solution products. To obtain technical information regarding Celvol or if you have a question regarding the sample ordering process, please can call our Product Information Center at 1-866-284-0927 or one of our regional contacts.


Instructions

1) Complete the first two sections of contact/delivery information.  Be sure to include your telephone and fax numbers, as samples cannot be shipped without this information. (Expect 3-5 days for domestic and 1-2 weeks for international deliveries)

2) Select the grades of Celvol you would like to sample (up to 3 samples allowed; for assistance choosing samples, view PVOH Product Line or call one of our regional contacts).

3) Include information concerning your application area, estimated sample testing timing, and current PVOH consumption details.

4) Click the [Send] button at the end of the form.
